By signing below, I acknowledge that I completely read this questionnaire, comprehend it fully, know that applying does not ensure approval and that untruthful answers or failure to comply with the requirements of this application or the foster contract can result in the forfeiture of any Paws4Progress animal fostered by me. I certify that the above information is correct, and I understand that the information will be verified.

I understand that by signing this form, I agree to release and covenant to hold harmless Paws4Progress Canine rescue and its members from any claims, damages, costs, or actions incurred as a result of the care or actions of the animal. I accept full responsibility for the animal(s) actions at all times, and release P4P from any liabilities or damages that may be incurred as a result of fostering such animal(s). I agree that the P4P is not liable under the concept of charitable immunity.

Liquidated damages:

Foster agrees to pay P4P the sum of $300 as liquidated damages in the event the terms of this contract are breached; this liquidated damage value being agreed to for the purpose of establishing value of the animal, and does not bar P4P from seeking return of the animal by judicial process or other legal means.

Attorney fees and court costs:

Foster agrees to pay reasonable attorneys fees and court costs in the event this matter is forwarded to an attorney for outside enforcement. In the event the undersigned breaches any of the above conditions, the undersigned may be subject to legal action including but not limited to damages and injunctive relief together with reasonable cost of said services.
